Restorative Engagement
As cognitive changes progress, the brain areas responsible for planning, sequencing, attention, and decision-making—often called executive function—can become more difficult to access. When these abilities are not gently supported, individuals may withdraw, lose confidence, or become frustrated during everyday activities.
Restorative engagement through From The Heart works by meeting the person where they are and encouraging participation in familiar, meaningful activities that stimulate these thinking pathways in a natural and supportive way.
Through one-to-one conversation, reminiscence, storytelling, music, art, shared tasks, and guided activities, individuals are supported to:
• participate in simple planning and sequencing
• engage attention and curiosity
• access long-held memories and skills
• experience success and purpose
• remain socially and emotionally connected
Rather than focusing on what has been lost, restorative engagement focuses on preserving abilities that remain and creating opportunities for meaningful interaction.
This approach is dementia-informed and non-pharmacological, supporting cognitive engagement and emotional well-being without relying on medication.
Sessions are calm, supportive, and adapted to the individual’s abilities and interests, helping to reduce frustration while encouraging participation and confidence.
Over time, these meaningful interactions can help support:
• continued engagement in daily life
• preservation of thinking pathways related to executive function
• emotional comfort and reduced distress
• a sense of identity, dignity, and connection
At its heart, restorative engagement supports the person behind the diagnosis, by creating opportunities to think, create, connect, and remain engaged in meaningful ways.
Personal Care Support
Care that Follows Relationship, Not Routine.
Our Person-Centered, at home Care Support is provided by a Certified PSW and grounded in the foundation of Restorative Engagement. We feel personal care begins with trust and connection, therefore, before care tasks and activities of daily living are attempted-time is taken to build trust, familiarity, and emotional safety through calm, one-on-one connection tailored to each person’s life history, personality, and comfort level. This relationship-first approach through our foundation of restorative engagement reduces distress, supports cooperation, and preserves dignity—especially for individuals living with dementia, frailty, or heightened vulnerability. Personal care may include assistance with bathing, dressing, toileting, mobility, meals, and daily routines- always delivered with patience, respect, and individualized pacing at home or retirement living settings.
**please note: Personal Care is not available in Hospital, Transitional Care or Long Term Care environments.


Responsive Behaviour Guidance & Delirium Risk Reduction
Changes in the brain can affect how a person experiences their surroundings.
Confusion, sensory overload, unmet needs, or sudden changes in health can sometimes lead to distress, agitation, withdrawal, or other responsive behaviours.
At From The Heart Supportive Care, we approach these moments with understanding and calm support. Rather than viewing behaviours as problems, we look for what the person may be trying to communicate. Through observation, environmental awareness, and dementia-informed interaction, we help identify possible triggers such as fatigue, overstimulation, changes in routine, or discomfort. By simplifying the environment, slowing interactions, and promoting calm connection, we can often help reduce distress and support emotional regulation.
We also remain mindful of factors that can increase the risk of delirium, a sudden change in attention and awareness that can occur with illness, infection, dehydration, medication changes, or hospitalization. Early recognition of changes in behaviour, alertness, or engagement allows caregivers and families to respond quickly and seek appropriate medical guidance when needed.
Our role is to support individuals and caregivers by:
• Observing changes in behaviour and engagement
• Supporting calm and emotional regulation
• Reducing sensory overload and environmental stress
• Encouraging routines and meaningful engagement
• Sharing observations with caregivers to support early awareness of delirium risk.
By understanding the changing brain and responding with patience and calm support, we help create an environment where individuals feel safer, more settled, and better able to connect with those around them.
Dementia Informed Caregiver Education, Support & Respite
Support Across The Care Continuum
Care under the Dementia umbrella can feel complex and overwhelming — especially as needs change across home, hospital, retirement living, long-term care, or end-of-life. Our Dementia Informed Caregiver education and support provides steady, relationship-based guidance to help families feel more supported, informed, and less alone.
We work directly with families to establish a baseline of behaviour and function, observing trigger patterns and sensory overload that help to establish practical, person-centered behaviour support plans (UCP-informed). FTH also assists with organizing documentation for care conferences and Ontario Health Team meetings, advocating for care that reflects the individual’s history, needs, and dignity.
Support may include respite care, non-clinical delirium screening through structured observation and reporting to the interdisciplinary team, behavioural guidance during illness or hospitalization, long-term care stabilization planning, medical appointment support, and navigation of community resources and funding options.
We also collaborate with care teams and staff to strengthen personalized approaches, reduce distress, and promote meaningful engagement based on identity, routine, and emotional safety.
As dementia progresses, we offer gentle support for individuals and families going through the emotional and practical transitions of care, including palliative and end-of-life dementia care integration.
This service is designed to bring clarity, stability, and compassionate direction — bridging gaps across the healthcare system with dignity and connection at the center and is included in all our supportive care services or as a stand alone service.
