For Healthcare Professionals
Thank you for visiting.
At From The Heart Supportive Care, we partner with healthcare professionals to provide restorative, person-centred support that builds upon and complements existing care. Our services are designed to help individuals preserve and maintain function through meaningful participation in everyday life while reinforcing the goals established by their healthcare team.
Delivered within the Ontario Personal Support Worker (PSW) scope of practice, Restorative Engagement is a non-clinical, evidence-informed approach that focuses on supporting function across the physical, cognitive, communication, emotional, behavioural, sensory, social, and practical domains of daily living. By integrating restorative principles into meaningful daily activities, we help individuals remain engaged in the roles, routines, and experiences that matter most to them.
Whether someone is recovering from stroke, living with a neurological condition, adjusting to a significant change in health, or experiencing functional decline, Restorative Engagement provides practical support that extends restorative care beyond clinical appointments and into everyday life.
Who May Benefit
Restorative Engagement may benefit children, adults, and seniors who would benefit from additional support to preserve, maintain, or maximize function through meaningful participation in everyday life.
Services are individualized and may be appropriate for individuals who are:
-
Recovering from stroke, acquired brain injury, or other neurological conditions.
-
Transitioning home following hospitalization, rehabilitation, or a significant change in health status.
-
Living with dementia or other cognitive disorders.
-
Living with Parkinson's disease, Multiple Sclerosis, ALS, Huntington's disease, or other progressive neurological conditions.
-
Living with developmental disabilities or complex medical needs.
-
Managing chronic illness, frailty, or age-related functional decline.
-
Recovering from orthopedic injury or surgery.
-
Experiencing reduced participation in meaningful daily activities due to physical, cognitive, communication, emotional, sensory, behavioural, or environmental barriers.
-
Seeking to maintain independence, confidence, family roles, and quality of life through continued engagement in everyday activities.
Every individual is unique. Support is tailored to each person's strengths, abilities, priorities, environment, life circumstances, and personally identified goals.
Why Refer?
Clinical care does not end when an appointment is over.
Many individuals benefit from ongoing opportunities to apply therapeutic goals within the context of everyday life. Restorative Engagement helps bridge the gap between formal rehabilitation and daily living by encouraging meaningful participation throughout ordinary routines and activities.
A referral may be appropriate when an individual would benefit from:
-
Reinforcing goals established by physiotherapy, occupational therapy, speech-language pathology, nursing, or other healthcare professionals.
-
Increased opportunities for meaningful participation between clinical visits.
-
Support incorporating restorative principles into everyday routines.
-
Encouragement to remain active within their current abilities.
-
Assistance navigating the transition from hospital or rehabilitation to home and community living.
-
Strategies that promote confidence, independence, and participation in valued life roles.
-
Functional observation during routine daily activities.
-
Caregiver education that supports restorative, person-centred approaches at home.
By extending restorative principles into daily life, individuals have additional opportunities to practise meaningful activities that help preserve function, maintain abilities, and support participation in the areas that matter most to them.
Collaborative Practice
Collaboration is fundamental to person-centred restorative care.
Restorative Engagement is designed to build upon and complement the care provided by regulated healthcare professionals. Services are delivered within the Ontario Personal Support Worker scope of practice and do not include assessment, diagnosis, treatment, or therapeutic intervention.
With informed consent, collaboration may include:
-
Reviewing referral information and identified functional goals.
-
Aligning restorative support with recommendations provided by the healthcare team.
-
Reinforcing established strategies during meaningful daily activities.
-
Providing objective observations regarding participation, engagement, functional abilities, and responses during routine care.
-
Communicating significant observations that may assist ongoing clinical decision-making.
-
Supporting caregivers through education and practical restorative strategies that encourage participation and independence.
-
Promoting continuity of care across healthcare, home, and community environments.
This collaborative approach helps ensure restorative support remains consistent with the individual's goals while promoting participation, dignity, and quality of life.
Referral Process
Referrals are welcomed from physicians, nurse practitioners, occupational therapists, physiotherapists, speech-language pathologists, nurses, social workers, care coordinators, hospitals, rehabilitation programs, community organizations, as well as individuals and family members.
Our referral process is straightforward:
Initial Discussion
We begin with a conversation to determine whether Restorative Engagement is appropriate for the individual's needs, goals, and circumstances.
Intake & Functional Observation
An intake assessment and functional observation are completed to better understand the individual's strengths, current abilities, daily routines, participation, and priorities.
Collaborative Planning
A personalized restorative support plan is developed that reflects the individual's goals while complementing existing recommendations from the healthcare team.
Ongoing Support
Services focus on meaningful participation, functional engagement, caregiver support, and preserving function through everyday life.
Communication
With consent, relevant observations and progress documentation will be shared with the referring clinician or healthcare team to support continuity of care.
