About From The Heart

From The Heart is a new, supportive care organization located in the Thousand Islands region of Eastern Ontario - created in response to a growing need for consistent, relationship-based support in aging and dementia care. Our model of care has been painstakingly designed to bridge the gaps between systems, routines, and real human experience.

​

With over 35 years experience in behavioral support and early cognitive development, it seemed only natural that our focus would eventually evolve into supporting individuals through aging, cognitive change, and dignified decline. It is this foundation that  guides our Restorative Engagement approach-helping adults navigate the aging process, feel grounded and remain understood as their needs shift across the care continuum. Our work is strengthened through continued Geriatric-focused learning, including studies in geriatric nursing through NYU Rory Meyers College of Nurses, Hartford Institute for Geriatric Nursing, McMaster University, and this coming spring; Laurentian University- all with an emphasis on cognitive change, behavioural expression, and compassionate supportive care.

A Restorative Engagement Approach

At the core of From The Heart Supportive Care services is Restorative Engagement—a supportive, one-on-one presence rooted in trust, familiarity, and emotional safety. Rather than focusing only on tasks, Restorative Engagement emphasizes the importance of supporting executive function, building confidence and connection, familiarity, fatigue pacing, and meaningful participation in routines that bring comfort- all tailored to each person's abilities in an effort to help individuals feel more secure, capable and engaged in their daily life. This approach helps reduce distress, support behavioural stability, and preserve comfort and dignity throughout the aging journey. The goal of RE is not to "restore" cognition, but to support an individual's executive function, confidence, and dignity for as long as possible. At From The Heart, we find Restorative Engagement is most beneficial when it is purposeful and personal; again-tailored to each person's history, strengths, and comfort.

Dementia-Informed and Behavioural Support Across the Continuum

Our dementia-informed supportive care is not only designed to support individuals and families living with dementia, Parkinson's , and Lewy Body- it  complements existing programs and care teams across care settings, including:

  • Home and community support

  • Retirement and supportive housing

  • Acute hospital environments

  • Long-term care transitions and ongoing life in care​

Care is guided by an understanding that behaviour is communication—expressions of unmet needs, fear, pain, confusion, or environmental overstimulation. Support is grounded in Ontario-aligned behavioural frameworks such as PIECES and BSO-informed care principles, always delivered with compassion and respect.

From The Heart was founded on the belief that the aging care deserves more than routines. It deserves connection, comfort and care that feels steady, and relational.

We are dedicated to providing more than just supportive care-we provide reassurance, advocacy, and guidance during complex moments........ and for older adults, that means being supported by someone who prioritizes dignity and emotional safety.
