The Heart Of What We Do

Through gentle, one-to-one connection, From The Heart supports adults living with Dementia, Parkinson's, Lewy Body Disease,, frailty, or complex aging. We focus on executive function support, emotional regulation, fall risk awareness, delirium risk observation, family education, reducing anxiety, strengthening comfort through familiarity, and helping each person feel more grounded and secure in their day-to-day care.

By providing care across the continuum — at home, in hospital settings, and through retirement living and long-term care transitions- we offer individualized personal care at home, restorative engagement, BSO-aligned behaviour expression documentation, and dementia Integrated care consulting designed to help families navigate care, funding options, behaviour communications, trigger mitigation plans, and complex care decisions with greater clarity and confidence.

During critical times of acute illness or hospitalization, FTH provides relational support that promotes orientation, comfort, and emotional safety — helping reduce distress and vulnerability to delirium; while documenting behaviour observations between each person's baseline patterns and acute behaviour expressions to help identify unmet needs and acute or unexplained behaviour expressions/triggers.

Finally, as supportive care requirements progress, we also provide compassionate presence, certified pastoral care (*where requested) and both personal and emotional supportive care through end of life transitions- ensuring individuals and families feel held, supported, and guided during difficult moments.
