Connect to C.A.R.E TM

Grounded in the philosophy that every person's story matters, the Connect to C.A.R.E.TM Framework is a model of person-centered, relational care designed to nurture the mind. body, and spirit. It reflects a belief that caregiving is not only about

doing for, but being with. This model integrates traditional community care practices with restorative and memory care ,dementia support, meaningful engagement, and social interaction-ensuring that caregiving is not simply clinical, but compassion based as well.

C- Connection

Connection is the heartbeat of the Connect to CARETM model. Beginning with

genuine presence, we take the time to see, listen, and share in moments of calm or confusion. Through connection; trust, relationships and security is built- allowing care to naturally flow from relationship rather than routine. Studies show, connection and emotional memory helps to sustain identity, belonging, and joy- long after functional memory fades.

A – Awareness

Awareness brings understanding and adaptability. In recognizing the emotional, physical, and environmental factors that shape a person’s day, awareness invites caregivers to pause,

notice, and respond with empathy — transforming challenging behaviors into opportunities for compassion, and ensuring care remains attuned, intentional, and individualized.

R – Restoration

Grounded in the philosophy that every person-regardless of ability-holds inherent worth, purpose, and potential, the Connect to C.A.R.E.TM framework focuses on preserving and maintaining both cognitive and physical function in an effort to maximize independence. This framework unites restorative care and memory support within a compassionate, relationship-driven model; encompassing practices that preserve dignity, mobility, and emotional balance.

E – Empowerment

Empowerment uplifts the individual, the family, and the care team.

By supporting choice, encouraging participation, and celebrating progress- no matter how small-the

Connect To C.A.R.E.TM method enlists families into partnership. As a result of ensuring that continuity, education,

and emotional support is prioritized, all those involved walk the care journey with confidence and grace.
